How to get a medical note
Step 1
Sign up and request a visit
Click "Get care" and let us know you need a medical note. Tell us what’s going on with your health. Then, you'll connect with a practitioner right away or schedule a time that works for you. They’ll do a video assessment to see if a medical note makes sense for you.
Note: Coverage varies. If you don't have employer or insurer coverage, standard visit fees apply.

Step 2
Connect with your practitioner
Your practitioner will review your request and start your video visit. They'll assess your symptoms and medical history to see if a medical note is right for you.
Medical notes work best for recent illnesses (within the past few days) and shorter absences. If you’re dealing with something more complex or ongoing, your practitioner may suggest seeing someone in person to make sure you're getting the care you need.

Step 3
Get your note
After your visit, you'll receive a digital PDF of your medical note, including your practitioner’s signature and contact information.

There’s no additional charge to get a medical note online outside of the consultation fee. Many doctors or nurse practitioners you’d see in person charge for medical notes, with prices ranging from $10 to $30. And, with Maple, you get to skip the waiting room at your doctor’s office or at a walk-in clinic.
