Maple Ontario Disclaimer

The Maple membership is available to residents of Ontario with a valid government-issued ID.

Your Maple membership covers virtual care for you, your spouse and your dependents under the age of 18 who share the same primary residence and includes one visit, per person, each day.

Prescriptions, sick notes, lab work and specialist referrals are given at the healthcare provider’s discretion.

We require a one-time intake call prior to your first general practitioner (GP) visit. Intake calls are available from 7am to 7pm EST, 7 days a week.

Maple membership payments are non-refundable. You can cancel your membership at any time and will not be charged for the next billing period. You will retain the benefits until the end of the billing period for your chosen membership.

The charges and credits you receive depend on the type of membership you choose as noted below.

Monthly Membership
When you join Maple, your membership will begin immediately. You’ll be charged $79.99 plus applicable taxes per month. Maple monthly members will receive an $80 credit each month to use towards any specialty visit. The credit does not carry-over or accumulate.

Annual Membership
When you join Maple, your membership will begin immediately. You’ll be charged $479.99 plus applicable taxes per year. Maple annual members will receive an $80 credit for the year to use towards any speciality visit. The credit does not carry-over or accumulate.

Maple is safe and effective for common non-emergency issues. Maple does not guarantee specific medical outcomes. If the provider thinks your condition requires in-person care, they may tell you to go to a clinic for a hands-on examination. If you believe you are experiencing an emergency, please call 911 or proceed to your nearest emergency room.
